Jan Revella, R.N.
  Jan Revella, R.N., Arthritis Nurse Specialist, is founder and director of Arthritis Education by Professionals, Inc. and one of the most prominent speakers on arthritis in the country. During the last 20 years, she has personally conducted education programs that have helped over 100,000 people throughout the United States. Her mission is to empower people with arthritis to use knowledge as power when making decisions about their personal healthcare.

Dennis Armstrong, M.D.
  Dr. Armstrong earned his undergraduate degree at Michigan State University and graduated from Wayne State School of Medicine in Detroit, Michigan. He served internships at Tripler Army Medical Center in Honolulu and Walton Army Hospital, Fort Dix, New Jersey. His orthopaedic residency was served at Henry Ford Hospital in Detroit. Dr. Armstrong is board certified by the American Academy of Orthopaedics, and has been in private practice in Arizona since 1977. He has participated in many design concept meetings and clinical studies of orthopaedic implants, especially total hip and total knee implant designs. Dr. Armstrong has also presented many original papers at national and international symposia on total joint replacement techniques.

Ralph Bennett, M.D.
  Dr. Bennett graduated from Mount Sinai School of Medicine in New York. He completed his internal medicine residency and rheumatology fellowship at Cleveland Clinic Foundation in Cleveland, Ohio. Dr. Bennett is board certified in internal medicine and in rheumatology. He has been a member of the medical scientific committee of the Arthritis Foundation and is actively involved in arthritis research projects. Dr. Bennett is associated with Arizona Arthritis & Rheumatology Associates, which has offices throughout metropolitan Phoenix, Arizona.

Culley Christensen, M.D.
  Dr. Culley Christensen received his medical degree from the University of Utah School of Medicine in Salt Lake City, Utah. He completed his orthopaedic residency at Maricopa County Affiliated Hospitals in Phoenix, Arizona. Dr. Christensen is board certified by the American Board of Orthopaedic Surgery. He is a diplomate of the National Board of Medical Examiners and a member of the International Society of Clinical Densitometry.Dr. Christensen is medical director of Affiliated Orthopaedic Specialists and is in private practice in Scottsdale, Arizona.

Dennis Crandall, M.D.
  Dr. Dennis Crandall received his medical degree from St. Louis University School of Medicine and completed his orthopaedic residency at St. Louis University Hospitals in St. Louis, Missouri. He completed a fellowship in spinal reconstructive surgery, adult and pediatric spinal deformity and spinal trauma from the University of Maryland, Section of Spinal Surgery, Division of Orthopaedic Surgery at the University of Maryland Hospital and Maryland Institute for Emergency Medical Services in Baltimore, Maryland. Dr. Crandall is board certified by the National Board of Medical Examiners and the American Board of Orthopaedic Surgery. He is a member of the American Academy of Orthopaedic Surgeons, American College of Surgeons, the Scoliosis Research Society, North American Spinal Society and the Spinal Fixation Study Group to name a few of his affiliations.

Michael Fairfax, D.O.
  Dr. Fairfax is a board-certified rheumatologist in private practice in Phoenix, Arizona. His special interests include rheumatoid arthritis, pediatric arthritis and osteoporosis. Dr. Fairfax graduated from the Kirksville College of Osteopathic Medicine; he served an internal medicine residency at Naval Hospital in San Diego and a rheumatology fellowship at St. Louis University School of Medicine in Missouri. Dr. Fairfax is a member of the International Society of Clinical Densitometry and the American College of Rheumatology, and holds teaching positions at the Kirksville College of Osteopathic Medicine--Southwest Campus and at the Arizona College of Osteopathic Medicine.

Jon Gelsey, M.D.
  Dr. Jon Gelsey received his medical degree from and completed his general surgery residency at the University of Illinois College of Medicine in Chicago, Illinois. He completed his orthopaedic residency at Wilford Hall, United States Air Force Medical Center at Lakeland Air Force Base in Texas. Dr. Gelsey is a fellow of the American Academy of Orthopaedic Surgeons. Dr. Gelsey specializes in treatment and surgery of the hip, knee and shoulder. Paul Howard, M.D.
  Dr. Paul Howard graduated from Georgetown University in Washington, D.C. He completed his residency in internal medicine at the Arizona Health Sciences Center in Tucson, Arizona, and his rheumatology fellowship at the John Hopkins University, Baltimore, Maryland. Dr. Howard is board certified by the American Board of Internal Medicine and Rheumatology. He is a fellow of the American College of Rheumatology, a national board member of the National Sjorgren's Syndrome Association and is a member of the American College of Physicians. Dr. Howard is Director of Arthritis Health, which promotes traditional and complementary therapies. He is a clinical lecturer at the University of Arizona Medical School in Tucson and teaches arthritis care to Family Practice and Internal Medicine Residents, and participates in clinical pharmaceutical studies of arthritis and metabolic bone disease.

Robert Karpman, M.D.
  Dr. Robert Karpman received his medical degree from the University of Pennsylvania in Philadelphia, Pennsylvania. He completed his orthopaedic surgery residency at the University of Arizona Health Sciences Center in Tucson. Dr. Karpman received his master's degree in business administration from the University of Phoenix, in Phoenix, Arizona. Dr. Karpman is a fellow of the American Academy of Orthopaedic Surgeons and the American College of Surgeons. He is also a member of the Association of Bone & Joint Surgeons. Board certified by the American Board of Orthopaedic Surgery, Dr. Karpman specializes in orthopaedic surgery, specifically the foot and ankle. He is an associate of Sun Valley Orthopaedics and holds academic positions including clinical professor, Department of Surgery, University of Arizona and as associate professor, Mayo Medical School.

Jonathan Komar, M.D.
  Dr. John Komar began his career at Wayne State University in Detroit, Michigan, where he earned his medical degree. He completed residency training in Physical Medicine and Rehabilitation at the University of Washington, one of the top programs in the nation. Under the guidance and supervision of Dr. Michael Wolff, he completed a year-long musculoskeletal and spine fellowship in Scottsdale, Arizona and now practices physical medicine and rehabilitation at Southwest Spine and Sports in Scottsdale and Mesa. Dr. Komar belongs to a number of professional organizations, including the Physiatric Association of Spine, Sports Medicine and Occupational Rehabilitation, the North American Spine Society and the American Academy of Physical Medicine and Rehabilitation. His personal area of interest includes musculoskeletal health, electrodiagnostic medicine and sports medicine. Dr. Komar is in private practice with Southwest Spine and Sports, which has locations in Scottsdale, Tempe and Mesa, Arizona.

Jonathan Landsman, M.D.
  Dr. Jonathan Landsman received his medical degree from Case Western Reserve University School of Medicine in Cleveland, Ohio. He completed his orthopaedic surgery residency at Mt. Sinai Medical Center in Cleveland Ohio and a spine surgery fellowship at the Florida Orthopaedic Institute in Tampa, Florida.
Dr. Landsman is a member of the American Academy of Orthopaedic Surgeons and the Arizona Orthopaedic Society to name only a couple of his professional affiliations. He specializes in neck and back diagnosis, treatment and surgery and is an associate of Phoenix Orthopaedic Consultants, which has offices in Sun City and Glendale, Arizona.

Anthony Lee, M.D.
Dr. Anthony Lee received his medical degree from Mercer University School of Medicine in Macon, Georgia. He completed his medical internship from the Medical Center of Central George in Macon and his physical medicine and rehabilitation residency at the Hospital of the University of Pennsylvania in Philadelphia, Pennsylvania. Dr. Lee completed a fellowship in musculoskeletal medicine and interventional spine at Southwest Spine and Sports in Scottsdale, AZ. After this fellowship, Dr. Lee joined the practice at Southwest Spine & Sports and continues in private practice. Dr. Lee is board certified in physical medicine and rehabilitation and is an affiliate member of the Physiatric Association of Spine, Sports and Occupational Rehabilitation and the American Academy of Physical Medicine and Rehabilitation.

Rene A. Lucas, M.D.
  Dr. Rene Lucas received his medical degree from the University of California School of Medicine in San Francisco, California. He completed his medical residency in physical medicine and rehabilitation at UCLA-VMAC West Los Angeles Program. Dr. Lucas is a diplomat of the American Academy of Physical Medicine & Rehabilitation and is board certified by the American Physical Medicine and Rehabilitation. He is the Medical Director of Del Webb's Acute Rehabilitation Unit and Sun Health's Industrial Rehabilitation Center.

Raymond Mirise, M.D.
  Dr. Raymond Mirise received his medical degree from the University of California in Irvine, California and completed his internal medicine residency at the University of Southern California Medical Center. Dr. Mirise completed a rheumatology fellowship at the University of Southern California as well. Dr. Mirise is board certified in internal medicine and in rheumatology. He is a fellow of the American College of Rheumatology and a member of the American College of Physicians and the American Society of Internal Medicine. Dr. Mirise is a practicing rheumatologist with offices in Glendale, just east of the Sun Cities in Arizona.

Eric Novack, M.D.
  Dr. Eric Novack received his medical degree from the University of California at San Francisco. He completed his orthopaedic surgery residency at the University of Washington in Seattle, Washington. Dr. Novack is board certified by the American Board of Orthopaedic Surgery and a member of the American Academy of Orthopaedic Surgeons. He brings a strong background in treating diseases and injuries in shoulders, hips and knees. He also has extensive training in fractures, ligament and overuse injuries. Blair Packard, P.T.
  Blair Packard received his Bachelor of Science degree from Brigham Young University in Provo, Utah and his Master's degree in Physical Therapy from Duke University in Durham, North Carolina. Blair is co-owner of East Valley Physical Therapy & Aquatic Rehabilitation in East Mesa, Arizona. Previously he was owner of Professional Therapy Associates in the same location as well as co-owner of Affiliated Physical Therapists in Phoenix. Blair is active in his profession at many levels including being past-president of the Arizona Chapter of the American Physical Therapy Association. He is currently president of the Federation of State Boards of Physical Therapy. He has previously been awarded Physical Therapist of the Year and the President's Award from the Federation of State Boards of Physical Therapy. Blair has contributed numerous papers for professional publications as well as providing multiple presentations to his peers and colleagues.

Warren Rizzo, M.D.
  Dr. Warren Rizzo received his medical degree from the Universita Degli Study di Padova near Venice, Italy. He completed his residency in internal medicine from Mercy Catholic Medical Center in Darby, Pennsylvania and a fellowship in rheumatology at Temple University Hospital in Philadelphia, Pennsylvania. Dr. Rizzo is board certified in internal medicine as well as in rheumatology. He is a fellow of the American College of Rheumatology and a member of the American College of Physicians. He is affiliated with Affiliated Orthopedic Specialists in Scottsdale, Arizona.

Sanford Roth, M.D.
  Dr. Sanford Roth is internationally known for his expertise and experience in the treatment and research of arthritis. Dr. Roth received his medical degree from Ohio State University in Columbus, Ohio. He completed a fellowship in rheumatology and internal medicine at the Mayo Graduate School of Medicine in Rochester, Minnesota. Dr. Roth is medical director of Arizona Education & Research. He is also Senior Consulting Rheumatologist for ArthroCare, Arthritis Care and Research, and Director of the Aging and Arthritis Program at Arizona State University. Dr. Roth is an authority on arthritis drug research and a consultant to the United States Food & Drug Administration office of Compliance Rheumatology and to numerous pharmaceutical companies. He is author and medical editor of major arthritis textbooks and journals. Dr. Roth is a founder and past president of the American Society of Clinical Rheumatology and the International Society for Rheumatic Therapy.

Paul Saiz, M.D.
  A native of the Valley, Dr. Paul Saiz obtained his undergraduate degree from the University of Notre Dame which included 6 months of study in Mexico City. Dr. Saiz received his medical degree from Baylor College of Medicine in Houston, Texas after attending on a full scholarship, and completed his orthopaedic residency at the Phoenix Orthopaedic Residency Program in Phoenix, Arizona. He went on to complete two fellowships, one in Spinal Reconstructive Surgery at Sonoran Spine Center in Phoenix and another in Musculoskeletal Oncology at Rush Presbyterian-St. Luke’s Medical Center in Chicago, Illinois. Dr. Saiz is board certified by the American Board of Orthopaedic Surgery. He is a member of the North American Spine Society, the American Academy of Orthopaedic Surgeons, the Arizona Orthopedic Society and the Musculoskeletal Tumor Society. Dr. Saiz specializes in diseases and disorders of the spine, musculoskeletal oncology, tumors of the spine, spine trauma and is certified in percutaneous reduction of vertebral fractures (Kyphoplasty).

Douglas Slaughter, M.D.
Dr. Slaughter is originally from Cincinnati, Ohio. He did his undergraduate work at Columbia Union College in Maryland before transferring to Ohio State University. He pursued a double major at Ohio State, studying both mechanical engineering and biochemistry. Dr. Slaughter earned his medical degree from the College of Medicine at the University of Cincinnati. He completed his surgical internship and orthopaedic surgery residency at the Phoenix Orthopedic Residency Program, working at St. Joseph’s Hospital, St. Luke’s Hospital, the Mayo Clinic, and Maricopa Medical Center. Following his residency, he completed a fellowship in spinal reconstructive surgery at Beth Israel Hospital in New York, working with internationally known spine surgeons. Dr. Slaughter is board certified in spine surgery and is a member of the North American Spine Society and the American Academy of Orthopaedic Surgeons. Dr. Slaughter enjoys all aspects of spine treatment and has specialized training in minimally invasive surgery including minimally invasive fusion surgery.

Monte Swarup, M.D.
  Dr. Monte Swarup received his medical degree at the University of Pittsburgh School of Medicine in Pittsburgh, Pennsylvania. He completed his residency program at Baylor College of Medicine in Houston, Texas. Dr. Swarup is board eligible by the American Board of Obstetrics and Gynecology and is a Junior Fellow of the American College of Obstetrics and Gynecology as well.
